PGIMER Chandigarh Post Doctoral and PG (MD/ MS/ MHA/ MSc/ Certificate) courses for 2013 session
The candidate is required to download the Application Form/Challan Form from the PGI
website i.e. http://pgimer.nic.in. After filling the application form, the candidate is required to
send it by Speed Post/Registered Post to Registrar PGI, Chandigarh on or before 06.10.2012.
After depositing the fee with the branch of State Bank of India, the candidate shall send a
PGI’s copy of the Challan Form along with the application form and retain a candidate’s copy
of Challan with him/her for future use. The third copy of the Challan form will be retained by
the State Bank of India. The Challan form is required to be filled up in the name of Director,
PGIMER, Chandigarh (Examination) in the Power Jyoti Account No. 32211613319 and those
candidates who have SBI account can also pay their fee through i-collect service of SBI and Icollect Account No. is 32222954695. In that case the INB reference No. will have to be
mentioned instead of Challan No. in the application form.
The fees for GENERAL/ OBC/OPH candidates is Rs.1000/- and for SC/ST Rs.800/- (indicating category i.e. General or SC/ST). However, the fees for DM/M.Ch examination is Rs.1000/-. (The candidates should write his/her Name, Address, contact number and course name in which he /she apply, on the back side of Challan form).
2. The candidates completing internship after 31.12.2012 are not eligible for course No. 1, 2
and 3.
3. The number of seats wherever indicated are provisional and may increase/decrease without
any prior notice.
4. DM/M.Ch - A candidate applying for more than one subject/course is required to submit
separate application form complete in all respect for each subject/course.
5. The candidates applying for DM/M.Ch courses can apply maximum up to two subjects in
separate application form.

House Job/(OHS): 02.12.2012 (Sunday)
DM/M.Ch/MHA: 13.12.2012 (Thursday)
